Beginning 8/15/17, Sapphire Pediatrics will begin our new Credit Card on File policy for all patients.
Sapphire Pediatrics has decided to implement this new policy to:
1. Create a faster check-in and check-out process for our patients
2. Easier payment process for copays, balances, and deductibles both in office and through our billing company
3. Eliminate paper waste

NOTE: Your credit card information is not kept on file in this office.  It is on a secure website called a gateway, and this office does not have access to the full credit card number once it is entered into the system the first time.  This gateway is more secure than having your card scanned each visit.

Lactation Services!! 
We are very pleased to announce in-office lactation consultations, provided by Dr. Sarah Pilarowski.
Dr. Pilarowski has completed the coursework for the International Board of Certified Lactation Consultants (IBCLB). 
Call our office for details or to schedule an appointment.

             Monday, Wednesday, and Thursday
Dr. Larabee is available 3 days/week
            Tuesday, Wednesday, and Friday
Dr. Pilarowski is available 3 days/week  
            Monday, Tuesday and Thursday

            Mondays, Tuesdays, Wednesdays, Thursdays, and Fridays
Amy, CPNP is available 2 days/week

When calling to schedule your appointment, please let us know if you have a provider preference for that appointment.  We will do our best to accommodate your request!
Influenza Vaccine Updates
We recommend flu vaccine for ALL of our patients.
Patients at particularly high-risk for complications due to flu illness include:
  • Children who were premature babies
  • Children with a history of asthma
  • Children with heart disease, or other serious chronic medical conditions
Flu Vaccine Rules:
  • Children over age 6 months may receive a flu vaccine.
  • Children under age 9 who are getting their very first flu vaccine must get 2 doses, at least 30 days apart.

Please call to make an appointment for your child at one of the above flu shot clinics.  Appointments for flu shot clinics are not available on ZocDoc.

Camp and School Forms Process
  • Please remember, we process many, many, MANY requests for forms daily.
  • Please allow 5-7 days for us to complete and send your forms.
  • Please fax or mail your forms to the office, and include a return fax number or addressed/stamped envelope for us to send completed forms.
  • We can send forms via email if you have signed a email consent form.
  • Forms will be completed using data from your child's last "well child check". If that visit was more than a year ago, your child needs a "well child check" before we can complete forms. A check-up can usually be scheduled within 2 weeks-- call for an appointment.
  • If your child has a check-up scheduled in the near future, bring your forms to the appointment. We will do our best to fill them out during the appointment.
